ScottsdaleA care guide, in plain words

Find a route to care

Check the person, then the record.

When this comes up

You have the name of the person who will treat you. A badge on a treatment page is not the record you came to find.

Start with the role beside the name. Use the matching board's own site. Keep a note of what the record actually says.

How to check the person treating you

  1. Get the full name and role

Ask for the treating person's full name, job, and licence details. Keep that information beside the treatment page while checking.

  2. Choose the matching board

Use the board for the stated role in the links below. Search the full name; use the licence details to distinguish similar names.

  3. Read what is listed

Check that the name and job match the record. Note whether the licence is current. Open any documents showing the board's decisions about the person's licence. These are board orders, not orders for medicine. Ask the board to explain any gaps or words you do not understand.

Which board goes with which letters?

Doctors may have MD or DO after their names. For an MD, use the Medical Board's Doctor Search. For a DO, use the DO board's Find your DO search.

For nurses with advanced training, Arizona's Nursing Board links to licence and certificate checks. For a physician assistant, use Arizona's PA board.

Keep the limits of the record in view

Some Arizona board records have limits on what appears online, as of October 6, 2026. An empty history cannot establish that no complaint or action ever occurred.

More public records may be requested; what is available depends on the type of record.
